Advance healthcare planning
Advance Healthcare Directives and Living Wills in East Cleveland
A healthcare directive should do more than name someone to speak for you. It should also help that person understand what matters to you. Planning can bring your care preferences and agent designation into one coherent set of instructions.
Compare the two parts of the decision
An advance healthcare directive can address both who may act as your healthcare agent or alternate and the healthcare preferences you want to communicate. A living-will instruction is part of that broader conversation about care preferences; naming an agent addresses who may make decisions on your behalf. Consider both questions together rather than assuming one answers the other.
Ask whether your written preferences give your chosen agent enough context to speak for you, and whether the person you name understands the role you intend. A healthcare agent’s authority is not the same as financial authority under a power of attorney. The scope and limits of any power of attorney depend on its document; one should not be assumed to grant every power.
Make a one-page care-preferences worksheet
Before preparing or revising documents, write a short worksheet in your own words. It is a conversation aid, not a substitute for the directive. Keep it specific enough to guide a discussion, but avoid trying to predict every possible medical situation.
Include:
- What matters most to you when considering treatment and quality of life?
- Which situations or outcomes would you want your decision-maker to understand are especially important to you?
- Who would you want asked to act as your agent, and who might be an alternate?
- What questions or uncertainties do you want to discuss before finalizing instructions?
Check older documents against your current choices
Gather any existing advance directive, living-will instructions, healthcare power of attorney, and related records. Compare the names of agents and alternates, the preferences expressed, and any differences in wording or scope. An older document may not reflect your current wishes, while a newer document may not address every instruction you meant to retain.
Also identify documents that serve different purposes. A durable financial power of attorney concerns financial decision-making; a healthcare directive concerns healthcare choices and the person designated for that role. A HIPAA authorization concerns access to health information and should not be treated as the same thing as authority to make healthcare decisions.
Prepare for a focused planning conversation
Bring your worksheet, copies of existing documents, and a short list of questions. Useful questions include: Do my written preferences give my agent practical guidance? Do the agent and alternate designations match my current choices? Are there inconsistencies between this directive and other documents? What should I share with the people I have named?
Tennessee’s Advance Directive for Health Care form includes an agent and alternate, healthcare preferences, and instructions about signing the document and sharing copies. Because this page does not prescribe execution steps, ask about the applicable process for your circumstances before signing or relying on a form.

Questions to bring to the discussion
If I name a healthcare agent, do I still need to explain my preferences?
The designation identifies who you want to act in the healthcare role; your preferences help communicate what matters to you. Tennessee’s advance directive form addresses both, so consider them together.
Does my healthcare directive let my agent manage my finances?
Do not assume so. Healthcare decision-making and financial authority are distinct roles. A power of attorney’s scope depends on its document, and a financial power of attorney should not be treated as a healthcare directive.
